Support Officers, Home Care – Chatswood and Surry Hills Offices
Our
Home care division is experiencing rapid expansion. The job will entail
visiting clients in their homes across metropolitan Sydney
Role Description:
•Work with clients and stakeholders in
planning and managing services;
•
Facilitate and provide effective case management to a
portfolio of L1 and L4 clients, clients with low complexity, receiving Home Care
Package or through other programs and service agreements;
•
Manage implementation of service delivery to clients in
accordance with client agreements and budget parameters whilst maintaining high
levels of customer satisfaction;
•
Work collaboratively with clients, their carers,
family, and/or advocates as well as other Home Care Service staff, health
professionals and other external stakeholders to maximise
clients’ independence and their ability to remain living in the community;
•
Understand and be able to articulate Home Care service
offerings to existing and prospective clients, family/carers,
client advocates and referral providers;
•
Undertake person-centred care
planning in accordance with Australian Chinese Community Association model of
care, developing care plans, budget, schedule of services, including
identifying client care needs, requirements and setting client service
expectations;
•
Ensure a strong understanding of client needs, requirements
and potential by monitoring the client’s circumstances and maintaining relevant
client records, care plans, schedule of services and budget.
Essential skills and characteristics
•
Current NSW Driver’s License;
•
High level of interpersonal skills
including the ability
to develop and maintain relationships within a team, with
customers, across service providers and with various stakeholders;
•
High level of problem-solving skills, to
successfully identify problems;
•
Strong verbal and written communication skills.
Desirable skills and characteristics
•
Tertiary qualification in Nursing/Social Work
or other Allied Health qualifications;
•
Ability to speak Cantonese and/or Mandarin;
•
High level of computer literacy, including
Microsoft word, excel and Power-point;
•
Ability to multi-task and understand key roles
within the organisation;
•
experience in Aged Care Industry within Home Care/Community
services sector
